The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long term, participative relationships.
Earning PCMH recognition is a distinction in which a practice can take pride. The NCQA Patient Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduced cost. About Pariksith Singh, MD
Pariksith Singh, MD, is board-certified in internal medicine. He received his medical education at Sawai Man Singh Medical College in Rajasthan, India, where he was awarded honors in internal medicine and physiology. His residency training occurred at All India Institute of Medical Services in New Delhi, India, and Mt. Sinai Elmhurst Services in Elmhurst, New York. Upon completion of his residency Dr. Singh relocated to Florida and worked several years before establishing Access Health Care, LLC, in 2001.

About Maria Scunziano-Singh, MD
Maria Scunziano-Singh, MD, is board-certified in internal medicine and is a Diplomate of the American Board of Internal Medicine. Dr. Scunziano-Singh began her medical career in 1982, as a respiratory care practitioner. This steppingstone took Dr. Scunziano-Singh toward the path to her medical degree, which she earned from New York Medical College in 1993. Dr. Scunziano-Singh’s residency training occurred at the highly prestigious Mt. Sinai School of Medicine. During her education Dr. Scunziano-Singh received the Joan M. Liman, MD, Award for Allied Health Professional Entering the Field of Medicine and the Enrico Fermi Scholarship/Certificate of Merit.
